Hawaii Home Care Agency Complete Startup Package

Prepare ownership, administrator and supervisor information; tax clearance and good standing; service descriptions, brochure and client contract; policies; and the island or areas you intend to serve. Use OHCA's application checklist for the complete submission.

The OHCA checklist includes pre-hire background clearance, including the Fieldprint determination, and TB documentation for employees and volunteers. It also asks for personnel records and orientation and training procedures.

The Hawaii Home Care Agency fee is $1,500 for initial licensure and $1,000 for license renewal, as listed in the initial and renewal fee schedules in HAR Chapter 11-103.1.
